Ultrasound diagnostics of liver fibrosis in children using point shear wave elastography
Abstract
The study is devoted to the assessment of the diagnostic accuracy of the point shear wave elastography (pSWE) method in determining the stages of liver fibrosis in children. The retrospective study included 15 patients (7 boys and 8 girls) aged from 6 months to 17 years with various chronic liver diseases. All children underwent pSWE on the SIUI Apogee 6500 ultrasound device and laparoscopic liver biopsy, after which the fibrosis stage was assessed according to the METAVIR scale. Statistical analysis showed a strong positive correlation between the shear wave velocity according to pSWE and the degree of fibrosis (r = 0.850, p = 0.0001). ROC analysis demonstrated high accuracy of the method: AUC was 0.96 for F≥2, 0.94 for F≥F2-F3, 0.86 for F≥F3-F4 and 0.86 for F=F4. However, in 33.3% of cases, discrepancies were found between the pSWE results and histology. False-positive results were observed in patients with high hepatitis activity, and false-negative results were observed in patients with Crohn’s disease. Thus, pSWE demonstrates high diagnostic accuracy and can be used as a screening method for assessing liver fibrosis in children, but data interpretation requires taking into account the clinical and laboratory context.
